What is a Widal Test?
A serological test that identifies antibodies produced in response to Salmonella infection. The results are given as titers indicating antibody concentration.
Purpose of the Test:
Diagnose typhoid and paratyphoid fever
Evaluate prolonged fever, fatigue, or abdominal discomfort
Support diagnosis when blood culture is not feasible
Distinguish between active infection and past exposure (with caution)
Who Should Get This Test?
Individuals with prolonged fever (>4–5 days), headache, and weakness
People with digestive symptoms such as abdominal pain or diarrhea
Individuals in typhoid-prone areas
Patients suspected of enteric fever but without access to culture tests
Components of the Test:
TO (Typhi O) antibody titer
TH (Typhi H) antibody titer
May include Paratyphi A & B titers depending on the lab
Understanding the Test Report:
High O titer → Suggests active infection
High H titer → Indicates past infection or vaccination
Titers like 1:160 or above are often considered significant, but interpretation depends on region and symptoms
Confirmatory tests or clinical correlation may be needed
